Child Waiver

In consideration of being allowed to participate in volunteer activities with Kids Kupboard, I, [Parent/Guardian Name], on behalf of myself and my child, [Child's Name], hereby acknowledge and agree to the following terms and conditions:

1. Supervision Responsibility: I understand that my child, [Child's Name], will need to be supervised by me or another responsible adult at all times during volunteer opportunities with Kids Kupboard. I agree to be present and responsible for my child's actions and behavior.

2. Staff Responsibility: I understand that Kids Kupboard staff members are not responsible for supervising or monitoring my child during volunteer activities. It is my responsibility to ensure my child's safety and well-being while participating in activities organized by Kids Kupboard.

3. Termination of Participation: I acknowledge that Kids Kupboard reserves the right to terminate my child's participation in volunteer activities if their behavior is deemed inappropriate or disruptive. Kids Kupboard staff members have the discretion to take appropriate action to address any misconduct or violation of rules.

4. Assumption of Risk: I understand and acknowledge that there are inherent risks associated with participating in volunteer activities, including but not limited to the handling of food, potential accidents, and interactions with other participants. I voluntarily assume all risks related to my child's participation in activities organized by Kids Kupboard.

5. Release of Liability: I hereby release, waive, discharge, and covenant not to sue Kids Kupboard, its officers, directors, employees, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child while participating in volunteer activities, whether caused by negligence or otherwise.

6. Emergency Medical Treatment: In the event of any injury or medical emergency involving my child during volunteer activities, I authorize Kids Kupboard staff members to seek and consent to emergency medical treatment as deemed necessary.

7. By signing this hold harmless agreement, volunteers agree not to hold Kids Kupboard liable for any injuries or damages that may occur during their participation in volunteer activities. Volunteers acknowledge that they are participating voluntarily and assume all risks associated with their involvement.

I have read and fully understand the terms of this Child-Youth Liability Waiver, and I voluntarily agree to its contents on behalf of myself and my child, [Child's Name].
